A CHIC or Canine Health information Center form is shown below.  Each breed club selects which tests are needed to be done--and how often--for a dog to get CHIC certified.  Dogs which have this form have had all the tests required by the individual breed club done.   They don't necessarily have to pass the tests however.  For more information on CHIC, click HERE.
